The Colorado Center for Nursing Excellence is excited to partner with the Colorado Community Health Network (CCHN) to offer this unique leadership development opportunity for Community Health Center (CHC) frontline staff. This program is built upon three years of successful extensive leadership development training for administrative and clinical staff within the clinics and is focused on supporting leadership development for employees with or without a leadership title. This program is designed to support leadership and skill development that are key components in supporting the CHC’s transformation to patient-centered medical home (PCMH).

What will I learn in this Program?
This program is a three month program that includes a three-day workshop that is followed by a series of three monthly group coaching calls to support the integration of leadership skills. This is a highly experiential program where participants are engaged in active learning strategies focuses at improving content retention and leadership motivation. Participants are expected to complete a preparatory assignment in order to be able to fully engage in the activities. During the three-day workshop, participants will receive training on the following topics:

Each leader will leave the three-day workshop with a complete toolbox of leadership strategies to engage in being a more effective team member and leadership within the clinics. All content has been identified by Community Health Centers as core elements in supporting Patient Centered Medical Home (PCMH) transformations and can enhance employee confidence and competency.
In an effort to support the integration of workshop content, participants will be asked to participate in weekly personal reflections and group coaching teleconferences. During the class each participant will sign-up for the follow-up coaching teleconferences that will be held once a month with a trained coach. These conference calls are intended to support leader integration of the skills from class and provide the leaders an opportunity to network with their peers to support ongoing growth. Calls are held for one-hour once a month in groups of 6-8 participants in order to provide each leader with individualized support.
When is the CCHN Emerging Talent Leadership Program being held?
The next 2016 CCHN Emerging Talent Leadership three-day workshop will be held April 13-15, 2016 At the Colorado Center for Nursing Excellence, Denver, CO. Participants must attend all three days of the workshop, for the curriculum is integrated and builds on itself over time. Participants are expected to participate in one coaching call per month for three months following the classroom content. Workshop days will be held from 7:30 am – 4:30 pm and participants are expected to attend the entire day. There is an administrative fee of $110 to attend the CCHN Emerging Talent Leadership Workshop. While this class has been designed specifically for frontline employees seeking to gain leadership skills, any employee within the CHC’s is welcome to participate. The program was specifically designed for: medical assistants, front desk, behavioral health specialists, outreach workers, patient navigators, PCMH leads and other staff who have had limited opportunities to learn leadership, communication and team-building skills outside of a work setting. This training is also aimed at staff who need additional skills to support PCMH transformations and high-achieving staff ready to make a difference at CHCs.
What will I need to do to prepare and expect during the Program?
Participants will be responsible for completing a personal profile assessment (DISC) and reading a short book prior to coming to workshop. These two homework items will be sent to participants at least two weeks before starts. Also, there will be minimal homework assignments on each day of the workshop. Participants will also have weekly reflective practice journaling and a series of three check-in calls over the three months following the class to help integrate concepts and grow their leadership.
CCHN Emerging Talent Leadership Program Registration and Financial Policies
The administration fee for the 2016 CCHN Program will be $110. Registration is not considered complete until payment is received. The due date for payment will be one week before the workshop. If payment has not been received by that date, the registration will be released. If payment is received in the last week AND there is space available the participant may join the class. If payment has not been received by the day of the class, the participant will not be allowed to attend, even if space is available. Cancellation that occurs less than 30 days but before the first day of the course will incur a $25 cancellation fee.
Attendance
Participants must be present for all hours of the workshop and are expected to participate in follow-up teleconferences. For a “no-show” or if a participant misses more than 2 hours, they will not be allowed to continue in the workshop. There will be no partial make-up arrangements made and no refund of tuition funds will occur
